The Field Level Hazard Assessment (FLHA) is a mandatory safety document used on all RedBlue job sites. Every crew must complete this assessment before starting work, ensuring hazards are identified, evaluated, and controlled at the field level.

This FLHA is provided by Coastal Safety Services Ltd. and forms part of RedBlue’s Occupational Health & Safety program.

What This Form Covers

1. Project Information

You will record:

  • Date, time, and site/project

  • Prime contractor

  • Scope of work

  • Assessment completed by / reviewed by (with signatures)

2. Task List

List all tasks to be performed that day.
Mark any Critical Tasks using the CT checkbox.

3. Hazard Assessment

For each task, document:

  • Personnel performing the work

  • Hazards present or potential

  • Priority Rating (PR)

  • Mitigation steps (SWP or SJP)

The form explains how to calculate PR using Exposure Frequency and Potential Outcome, with ratings from Low to High.

How to Use This Form

  1. Complete the FLHA at the start of every shift.

  2. Identify hazards early and assign accurate PR values.

  3. Attach SWPs or SJPs for any critical tasks.

  4. Review and sign before work begins.

  5. Keep the completed form accessible on site.
